One Patient at a time

Only one adult may accompany any minors

Pre-filled Forms

Please fill out patient forms ahead of time

Cleaning / Disinfecting

Our staff will clean and disinfect all equipment and surfaces after each exam

Locked Doors

Doors will be locked to properly control social distance.  Please use front glass door for entry.  Back door remains closed.
